No. Services currently available are offered as walk-ins during opening hours. CT and MRI are coming soon and are not yet offered.
A referral letter or request form from your doctor. Valid photo identification, and a valid insurance card - a photo on your phone is accepted for both. Any sample you were asked to collect, in its container. Two things worth checking before you leave the doctor: on an SZF form the stamp, the signature and the arts-szf-code must all be clearly legible, and a PZS or Parsasco form must carry the doctor's stamp.

Yes, as long as you can name the determinations you want. Bear in mind that a test you request yourself is at your own expense and is not reimbursed by insurance.
At home. Do not wrap it in toilet paper - transfer it straight into the container.
Keep the container cool and bring it in on the next working day.
No. It must be collected at home or another suitable place, and delivered to the laboratory within one hour.
It differs. Men: do not urinate for at least three hours before the test. Women: do not come during your period, and use no creams, ointments or intimate lotions on the day.
No lotion, cream or oil on your skin. No jewellery. Wear clothing you can take off easily.
It depends on the area being scanned. Upper abdomen: come fasting - no food, though a few sips of water is fine. Lower abdomen: you need a full bladder, full enough that you feel the urge to urinate.
